Educational program and Tele-nursing effects on Glycemic control, Metabolic parameters and Adherence in adults with type II diabetes Mellitus

Background:   Effective care and control of diabetes cannot be achieved without continuous follow-up between patients and health professionals. Prevention of diabetic complications requires good Glycemic control. Adherence is a critical factor conditioning the success of diabetic managements. The aim of this study is to evaluate the effects of educational program combined with 3 months telenursing (nurse‑ telephone follow-up calls) on Glycemic control, metabolic parameters and adherence to diabetic recommendations in patients with type 2 diabetes. Methods: In this clinical trial, 60 adults  patients with type 2 diabetes who attending the diabetes clinic selected and were randomly assigned to study and control groups. Educational program was specified for study group (n = 30) and that they were followed up by telephone follow-up calls for twelve weeks by the researcher. The control group (n = 30) received the traditional diabetic management. Socio-Demographic characteristics, adherence with diabetic treatment regimen (diet, drug use, exercise, and follow up, self monitoring of blood glucose) and Glycemic control and metabolic parameters were recorded before and after interventions. Results: There was a statistically significant improvement in   Blood glucose indices (FBS, PPBG, Hb A1C, total cholesterol, LDL and HDL) in study group after intervention P ˂ 0.001. While no significant differences between the two groups in triglyceride level and systolic, diastolic blood pressure.  During study marked improvement in adherence percentage. Patients with good compliance in study group changed from 10% to 70%, after intervention, whereas good compliance in control group changed from 0% to (3.3%)  (P ˂ 0.001). Conclusions: According to the results of the current study educational intervention followed by12 week's telephone follow-up call by a nurse leads to significant improvement in Glycemic control, metabolic parameters and adherence to diabetic treatment regimen.
